Job Description

Tender Home Health is hiring licensed Physical Therapists across the San Francisco Bay Area for Full-Time, Part-Time, and Per Diem positions.

We are looking for clinicians who value autonomy, flexibility, strong team support, and meaningful patient care — without overwhelming territories or rigid scheduling.

Coverage Areas

We are actively hiring throughout the Bay Area, including:

• San Francisco

• San Mateo County

• Marin County

• Alameda County

Territories are assigned strategically based on clinician location whenever possible to help minimize unnecessary driving and improve work-life balance.

What You’ll Do

• Evaluate and treat patients in their homes

• Develop individualized treatment plans and therapy goals

• Help patients improve mobility, strength, balance, safety, and independence

• Collaborate with physicians, nurses, case managers, and interdisciplinary teams

• Complete documentation and maintain compliance standards

• Manage your caseload independently while receiving ongoing team support

Schedule Expectations

• Flexible scheduling options available

• Combination of field visits and remote administrative/documentation time

• Full-Time clinicians typically complete approximately 35 field hours weekly with additional remote administrative responsibilities

Qualifications

Required:

• Active California Physical Therapist (PT) license

• Reliable transportation and valid driver’s license

• Strong communication and organizational skills

Preferred:

• Home health experience preferred but not required

• Experience in outpatient, SNF, acute care, or rehabilitation settings welcome

• Bilingual candidates encouraged to apply
