Position Summary

Life Health Care is seeking a compassionate, dependable, and clinically confident Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing healthcare team.

This position is particularly suited for a provider who is comfortable working in a fast-paced clinical environment serving individuals with severe substance use disorder (SUD), opioid use disorder, co-occurring behavioral health conditions, and complex medical needs .

Our patient population often requires timely clinical intervention, medication management, close follow-up, and coordinated care. Providers must be comfortable making sound clinical decisions efficiently while maintaining appropriate documentation, patient safety, professional boundaries, and compassionate care.

The position may be performed in person, virtually through telehealth, or through a hybrid arrangement . Both full-time and part-time candidates will be considered.

Essential Responsibilities

The NP or PA will be responsible for:

• Conducting comprehensive and focused patient assessments.
• Evaluating patients experiencing substance use disorder, including individuals with significant or complex treatment needs.
• Obtaining and documenting medical histories, symptoms, current medications, substance use history, treatment needs, and relevant risk factors.
• Diagnosing and managing acute and chronic health conditions within the provider's scope of practice.
• Developing individualized treatment plans in collaboration with patients and the interdisciplinary care team.
• Providing medication management for substance use disorder and other medical or behavioral health conditions within the provider's scope of practice.
• Prescribing medications when clinically appropriate and permitted by licensure, applicable regulations, and organizational policy.
• Providing ongoing follow-up and monitoring for patients receiving medication-assisted treatment or medications for opioid use disorder.
• Reviewing treatment adherence, medication response, side effects, laboratory findings, and other clinical indicators.
• Ordering and interpreting laboratory tests, screenings, and other appropriate diagnostic services.
• Responding promptly and appropriately to abnormal laboratory findings, medication concerns, changes in patient condition, and other clinically significant developments.
• Providing preventive care, health education, harm-reduction counseling, and risk-reduction education.
• Identifying patients requiring a higher level of care, emergency evaluation, specialty services, or additional behavioral health intervention.
• Delivering services through secure telehealth platforms when assigned to virtual care.
• Completing accurate, thorough, and timely documentation in the electronic medical record.
• Maintaining an efficient workflow while ensuring that clinical quality and patient safety remain priorities.
• Coordinating care with physicians, other advanced practice providers, nurses, medical assistants, therapists, recovery coaches, pharmacies, laboratories, and community partners.
• Making referrals to specialists, behavioral health providers, treatment programs, and community resources when appropriate.
• Participating in case consultation, quality-improvement activities, clinical meetings, and interdisciplinary treatment planning as needed.
• Maintaining appropriate professional boundaries while providing trauma-informed, respectful, and nonjudgmental care.
• Following HIPAA, confidentiality, prescribing, documentation, and clinical requirements.
• Following all applicable federal and Michigan laws, professional licensing requirements, and organizational policies.

Fast-Paced Substance Use Disorder Care

A significant component of this position involves working with individuals experiencing severe and potentially life-threatening substance use disorders .

Patients may present with complex circumstances that require the provider to prioritize multiple needs, make timely clinical decisions, and coordinate rapidly with other members of the treatment team.

Providers should be comfortable working with patients who may experience:

• Severe opioid or other substance use disorders.
• Relapse or recurrence of substance use.
• Medication interruptions or treatment instability.
• Polysubstance use.
• Co-occurring mental health conditions.
• Chronic medical conditions.
• Housing, transportation, financial, or other barriers to healthcare.
• Previous incarceration or involvement with the justice system.
• Difficulty consistently engaging with traditional healthcare systems.
• Rapidly changing treatment or recovery needs.

The successful provider will understand the importance of meeting patients where they are while maintaining appropriate clinical standards and expectations .

This role requires a provider who can move efficiently from one patient encounter to the next, communicate effectively with support staff, recognize urgent clinical concerns, and help prevent unnecessary interruptions in treatment.

Preferred Areas of Experience

Experience in one or more of the following areas is preferred but not required:

• Addiction medicine
• Substance use disorder treatment
• Medication-assisted treatment (MAT)
• Medications for opioid use disorder (MOUD)

• Family medicine

Providers who do not have extensive addiction medicine experience but demonstrate a strong interest in treating substance use disorder and a willingness to learn will also be considered.

Required Qualifications

Candidates must possess:

• Current and unrestricted Michigan Nurse Practitioner or Physician Assistant license.
• Graduation from an accredited Nurse Practitioner or Physician Assistant program.
• Current applicable national board certification.
• Current BLS or CPR certification.
• Ability to obtain and maintain professional liability coverage.
• Current DEA registration or eligibility to obtain DEA registration if prescribing controlled medications is part of the assigned role.
• Strong clinical judgment and clinical decision-making abilities.
• Strong communication and documentation skills.
• Ability to manage a fast-paced patient schedule while maintaining quality of care.
• Ability to work independently while collaborating effectively with an interdisciplinary clinical team.
• Ability to utilize electronic medical records and telehealth technology.
• Ability to communicate effectively with clinical and administrative support staff throughout the workday.
• Commitment to patient dignity, confidentiality, cultural responsiveness, harm reduction, and recovery-oriented care.
• Ability to successfully complete required background checks, credentialing, and payer enrollment.

Virtual-Position Requirements

Candidates providing virtual or telehealth services must have:

• A private and professional workspace that protects patient confidentiality.
• Reliable high-speed internet.
• Appropriate equipment to conduct secure telehealth visits.
• The ability to safely and effectively assess patients through telehealth.
• Availability to communicate throughout the workday with on-site clinical and support staff.
• The ability to respond promptly to clinical questions and patient-care coordination needs.
• Compliance with applicable telehealth, prescribing, documentation, licensure, and patient-location requirements.
