Esperanza Health Centers is seeking a Peer Recovery Support Specialist to join our care team in Chicago. You will provide case management for patients with substance use disorders and support recovery efforts within a patient-centered care model. You will collaborate with medical providers, counselors, and staff to develop recovery plans, identify relapse prevention strategies, and help patients access MAR services.
