Beyond clinical care, our Family Services team supports thousands of families across the region with programs that help meet essential needs and reduce barriers that impact health and well-being.At CHP, our mission is to provide access to health care for everyone.Salary Range: $104,500.00 - $120,000.00 / year The Family Nurse Practitioner will provide advanced primary and specialty nursing services. The NP performs patient health assessment, creating strategies to improve or manage a patient's health, and introducing habits for health promotion. Our expectation is that this position will see at least two patients per hour of patient facing time.This person will support CHPs mission, vision, and values and will adhere to compliance protocols as well as CHPs policies and procedures.POSITION REQUIREMENTSFamily Nurse Practitioner Essential Duties and Responsibilities:* Perform physical exams and patient observations.* Record patients' medical histories and symptoms.* Create patient care plans and contribute to existing ones. #J-18808-Ljbffr
