Physical Therapist - Travel Contract

Minimum Requirements Associate's, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Minimum education requirement: Associate Degree (client specific). Active Massachusetts Physical Therapy license required to start the assignment. Home health experience required. Oasis and homecare experience required (client specific). Experience using OASIS documentation and homecare workflows (client specific). BLS certification from AHA or ARC may be required. Interpersonal skills preferred. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Days 0800-1600. Typical scheduled hours noted as 8:00 AM to 4:30 PM in client notes. Weekly hours: Guarantee of 32 hours with availability of up to 40 hours based on caseload (client specific). Productivity expectations: 26 points in 32 hours or 33 points in 40 hours (client specific). Coverage Area: Travel to patient homes in Shrewsbury, Westborough, Northborough, Bolton, Hudson, Marlborough, and Framingham service areas (client specific). Mileage reimbursed between visits from first to last patient. On-call/Weekend Requirement: No regular on-call requirement. Occasional day swaps may be permitted for valid reasons (client specific).
