This role monitors eligibility, coordinates applications and recertifications, resolves benefit-related issues, and ensures compliance with program requirements. The position works closely with ACT teams, recipients, families, and community agencies to facilitate access to public and community-based resources.Essential ResponsibilitiesMonitor recipient eligibility and benefits, including Medicaid, Medicare, SNAP, SSI/SSDI, housing subsidies, and state and local assistance programs.Coordinate the full application and recertification process, including document collection, submission, tracking deadlines, addressing eligibility issues, and assisting with appeals.Conduct regular reviews to identify changes in eligibility related to income, housing, treatment status, or other relevant circumstances.Notify recipients and clinical teams of upcoming recertification deadlines and required documentation.Collaborate with ACT clinicians to align benefits and resource plans with individual treatment and recovery goals.Visit ACT program sites and participate in team meetings to identify barriers to accessing benefits and develop appropriate solutions.Transport and accompany recipients to DSS, HRA, Social Security Administration, and other agencies as needed.Educate recipients regarding benefit requirements, rights, responsibilities, and maintaining eligibility.Coordinate with recipients, families, and external agencies to resolve application delays, denials, and other benefit-related issues.Document client and collateral contacts, application updates, and services in the EHR within required timeframes.Develop treatment plans and coordinate case management services with ACT team members and recipients, as appropriate.Assist recipients with developing independent living skills, including budgeting, nutrition, transportation, community resources, shopping, housekeeping, vocational development, and social development.Serve as a resource and role model by helping recipients identify and access community resources and follow through on treatment or aftercare recommendations.Provide crisis intervention and mental health support within the scope of the position.Maintain accurate and timely documentation in Vellohealth.Participate in ACT supervision, required trainings, and benefit-specific training to remain current with eligibility requirements and application processes.Maintain confidentiality and report and document incidents to the immediate supervisor within 24 hours.Other ResponsibilitiesMaintain effective written and verbal communication with recipients, families, team members, and external providers.Work flexible schedules based on program, training, and client needs.Travel between multiple ACT program sites and manage an assigned caseload across programs.Operate an agency vehicle as required.Assist with team responsibilities during staff absences or periods of staffing shortages.Perform other duties as assigned by the Team Leader, Deputy Director, Division Director, or Senior Management.What You’ll GainCompensation: Competitive hourly rate based on experience.Robust Benefits: Medical, dental, vision, and 401k retirement plan (with matching).Work-Life Balance: Paid time off, holidays, and personal days.Wellness Program: Free and low-cost gym and wellness access and support.Training & Growth: Ongoing professional development and career advancement opportunities.Meaningful Work: Direct impact on the lives of youth and their families.Supportive Environment: A collaborative team that values your contributionsHigh school diploma or GED required, Bachelors in human services preferred. Minimum of 2 years’ experience working with individuals with mental illness required.TWO YEARS EXPERIENCE WORKING WITH MENTAL HEALTH AND SUBSTANCE ABUSE CONSUMERS PREFERRED.Experience working with behavioral health population preferred.Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.Valid New York State driver’s license and vehicle required – candidates are subject to regulatory background checks and must be willing to utilize own vehicle for home visits. #J-18808-Ljbffr WellLife Network
