Job Description – Home Health RN Case Manager DFW area (908875) Job Summary Provides direct nursing care, including patient observation and delivery of prescribed treatments. Conducts initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points. Evaluates outcomes of patient care and identifies discharge planning needs. Consults with other disciplines as required and adjusts nursing care processes as necessary. Maintains patient records and charts to ensure accurate records of treatment, including medication profile, care plan, communication notes, etc. Provides instructions to patients/families for use of products and services. Prepares, administers, monitors, and discontinues IV fluids and accesses IV delivery systems (e.g., peripheral IV start, implant cath, Broviac, etc.). Administers medications and treatments as prescribed by the physician. Performs phlebotomy, laboratory specimen collection, and point‑of‑care testing. Responsible for overall delivery of quality patient care that meets or exceeds established standards and provides assistance in activities to meet state and/or federal regulatory guidelines. Maintains a safe, comfortable, and therapeutic environment for patients/families. Participates in Continuous Quality Improvement (CQI) program and provides supervisory visits for Home Health Aides and LVNs as required by state and federal regulations. Participates in on‑call rotation and emergency call according to agency policy and performs other duties as assigned. Experience 2 years of RN experience with one (1) year of adult IV expertise required. Licenses and Certifications

• RN) Registered Nurse by Board of Nurse Examiners for the State of Texas (or BNE Compact State)
• Drivers) Valid Texas driver’s license with proof of driver’s insurance
• BLS) Basic Life Support accredited course by the American Heart Association (AHA) or American Red Cross (ARC)

Preferred Bachelor’s Degree – BSN Other RN experience including high‑tech and home health expertise preferred Knowledge of InterQual and TMF care criteria preferred Job Duties Provides direct nursing care and performs OASIS assessments.
