Social Work Associate

Responsibilities Work with clinical social worker to provide social work services at the service units Complete psychosocial assessment for patients as indicated Formulate and implement the intervention plan for patients Provide concrete case management, follow‑up and referral service Provide entitlements/benefits eligibility counseling and assist with application Assist Clinical Social Workers in providing case management service as needed Timely services provided to patients based on the Social Work department guideline Complete documentations and reports based on the Social Work department guideline Attend case conference, agency meetings and staff meetings Participate in community outreach and marketing activities Perform other duties as assigned Minimum Qualifications Bachelor's Degree in Social Science, Social Work, Psychology, Sociology or other related field At least one year of working experience in social services, health care agency or related field Good verbal and written communication skills Compensation and Benefits Salary: $50,000.00 - 55,000.00 per year Medical insurance Employer‑paid dental, vision, disability and life insurance 403(b) retirement savings plan Flexible spending accounts for transit, parking, health and dependent care Paid time off (28 days for the first year of employment) Employee Assistance Program Employee Discount Programs Tuition reimbursement for eligible employees The Health Center is qualified for Public Service Loan Forgiveness (PSLF) program #J-18808-Ljbffr Charles B.
