Job Description

Deliver primary care in a dynamic setting by performing home health risk assessments for Medicare Advantage plan members throughout Spotsylvania County. This per diem opportunity offers true flexibility, letting you tailor shifts around your life while making a genuine difference in patient care. You will visit members in their homes including areas such as Spotsylvania Courthouse and Lake Wilderness to complete annual assessments or conduct post-discharge check-ins. Each visit typically takes about 45 minutes, with all scheduling handled to fit your personal availability.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own working days and hours
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete annual and focused home health risk assessments for Medicare Advantage members
• Review and evaluate medical histories, ongoing health conditions, and medications
• Educate patients about their medical conditions and prescribed treatments
• Identify and address care gaps with actionable recommendations
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner (NP) license in Virginia
• Board Certification in good standing with AANP or ANCC
• Current CPR/BLS certification
• MSN from an accredited nursing program
• At least 6 months recent NP experience preferred
