Patient Care Manager

Artemis Healthcare Partners is seeking a compassionate and experienced nursing leader to join our client's home health team! Position Name: Patient Care Manager (PCM) Setting: Home Health Pay: $110,000-$135,000 Shift: Full-Time Qualifications: Required: Active and unrestricted Registered Nurse (RN) license in the state of California. Strong knowledge of accepted nursing standards, clinical best practices, and patient care principles. Understanding of Medicare Conditions of Participation and applicable federal, state, and local healthcare regulations. Demonstrated ability to provide clinical oversight, communicate effectively with multidisciplinary teams, and support consistent delivery of quality patient care. Preferred: Previous home health or home care experience. At least two years of nursing leadership, supervisory, or management experience. Experience coaching clinical staff, monitoring patient outcomes, and participating in quality improvement initiatives. Job Summary: Provide clinical leadership and supervision to ensure patients receive appropriate, consistent, and high-quality care in accordance with professional standards and regulatory requirements. Coordinate and monitor patient care plans while collaborating with clinicians, patients, caregivers, physicians, referral partners, and office personnel. Supervise clinical team members and help ensure services are delivered according to established care plans, organizational expectations, and patient needs. Serve as a resource to field staff by providing guidance, coaching, and clinical support related to patient care and daily operations. Promote clear and timely communication among patients, caregivers, referral sources, field clinicians, and internal team members. Monitor departmental performance and patient outcomes while participating in quality improvement efforts designed to strengthen clinical care and operational effectiveness. Support staff development through ongoing coaching, education, feedback, and continuing professional development. Maintain high standards of professionalism, service, accountability, and compliance throughout daily operations. Participate in required compliance activities, audits, investigations, and reporting processes while ensuring documentation and operational practices remain accurate and complete.
