Licensed Practical Nurse - Bronx (Monday-Friday)

Description We are currently looking for a full‑time LPN for one of our Bronx residential programs supporting adults with developmental disabilities, Monday through Friday. Job Type Full‑time Summary Under general supervision, the LPN is responsible for providing medical treatment, first aid, emergency care, and medication administration in accordance with ADAPT Community Network policies, regulatory agencies, and professional standards. Essential Duties and Responsibilities Serve as a member of the interdisciplinary team; assist the Registered Nurse in coordinating health care services for assigned residences. Keep progress notes under the supervision of the registered nurse for each client and summarize for quarterly, semi‑annual, and annual reviews. Provide health care to individuals receiving services as instructed by the registered nurse; offer direct hygiene and health instruction to individuals and families. Control communicable diseases and infections through identification, assessment, and reporting; implement protective and preventive measures and monitor infection control. Provide first aid to persons in need of immediate care and refer to the registered nurse or outside emergency services as necessary. Assist physicians in medical examinations as needed. Maintain a tickler for all appointments; schedule appointments and follow up on physicians’ recommendations. Keep accurate records of examinations, immunizations, illnesses, accidents, and other pertinent data via progress notes. Administer medications when appropriate; supervise medication storage and assist in ordering. Assist the registered nurse in maintaining AMAP re‑certification files. Obtain consent for surgery of residents in the absence of the registered nurse. Refer all consults and diagnostic reports to the registered nurse for review. Attend and transport clients to appointments as necessary. Attend team meetings as scheduled. Attend to clients’ medical conditions. Carry a beeper and be on‑call, responding to questions regarding physician and RN treatment plans. Keep abreast of changes in the nursing field. Perform other related duties as requested. Qualifications Ability to read, write, speak, and understand English. Good oral and written communication skills. Good organizational skills. Interpersonal skills necessary to interact effectively with coworkers, employees, the people we support, families, and outside contacts. Maintain strict confidentiality. Knowledge of equipment typically used by persons receiving services, including wheelchairs and adaptive devices. Willingness to work at various program sites. Education and/or Experience Graduate of an accredited professional nursing program. At least one year of experience as a Licensed Practical Nurse in community‑based health services to individuals with developmental disabilities. Licensed and currently registered as a Licensed Practical Nurse in the State of New York. Compensation $44/hour – $48/hour, plus industry‑leading benefits for all full‑time employees.
