Education Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from a recognized medical institution.
• Graduation from a certified Family Medicine residency program.

Certifications

• Board Certification in Family Medicine.
• Active medical licensure in Utah (or eligibility to obtain).
• BLS and ACLS certifications are advantageous.
