Primary Care Physician

Responsibilities Accountability for outcomes: Demonstrate accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in a panel of up to 450 patients. Coaching for health: Act as a health coach, help patients set short‑ and long‑term health goals, partner with them to work toward those goals, and frequently follow up on progress. Simplifying for action: Simplify and prioritize appropriately so that behavior change is more actionable, both for patients and for leading care teams toward performance goals. Deliver care for acute and chronic illnesses encountered in older adults; take full accountability for patient care and outcomes; seek specialist consultation when needed while remaining responsible for the detailed care. Engage with hospitalists whenever a patient is in the hospital. Assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. Lead your care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office. Coordinate transition teams and other resources for patients who are unable to come to the office (hospital, SNF, LTC, homebound). Participate in center management and cover for other providers when needed. Recruit patients for the center and additional providers for the company. Supervise, collaborate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the APP’s actions while employed with the company. Perform other duties as assigned and modified at the manager’s discretion. Knowledge, Skills & Abilities Availability and accessibility: Be open for walk‑in visits and answer phone calls/messages in a timely manner. Service orientation: Provide care that you would want for a family member or yourself to each patient at every interaction. Evidence‑based medicine: Stay updated on evidence‑based medicine and on lifestyle, nutrition, and other factors that influence patient outcomes. Physician leadership: Continuously develop and improve leadership skills for the benefit of patients, team, center, and company. Quality focus: Work with care teams to achieve high quality outcomes and quality measures; continuously improve practice and center operations. Influence: Competently and compassionately influence patients, teams, and self to achieve the best outcomes. Self‑care: Adequately care for yourself physically, mentally, socially, and spiritually to better care for patients. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other office software. Ability and willingness to travel locally, nationally for initial training (2 weeks) and occasionally regionally and nationally for recruiting or training purposes. Fluency in English, verbal and written. Fluency in other languages may be required depending on the center. Pay Range Salary: $221,141 – $315,915. Final compensation will depend on experience, education, geographic location, and other factors. The position may also be eligible for bonuses or commissions.
