Acute Rehabilitation Case Manager

This position offers a schedule of Monday through Friday, with hours from 8:00 AM to 4:30 PM.

Job Description

We are seeking an experienced Acute Rehabilitation Case Manager to join our state-of-the-art inpatient rehabilitation hospital. This role is critical for the treatment and recovery of individuals suffering from severe injuries or illnesses. You will utilize your clinical expertise and case management skills to advocate for patients and ensure optimal care throughout their rehabilitation journey.

Responsibilities

• Conduct comprehensive assessments and develop patient-centered care plans in collaboration with a multidisciplinary team.
• Facilitate family and team conferences to discuss care strategies and patient progress.
• Utilize review processes to ensure compliance with acute rehabilitation criteria.
• Maintain effective communication and coordination among nursing, therapy, and medical staff.
• Advocate for patients regarding care options and insurance issues.

Required Experience / Certifications / Licensure

• Current licensure as a Registered Nurse, Social Worker, or any healthcare professional with case management experience.
• A minimum of 2-3 years of experience in acute care or rehabilitation settings is preferred.
• Certification in Case Management or Rehabilitation Nursing is preferred, such as CCM, ARN, ACM, or ANCC RN-BC.
• Strong oral and written communication skills; competency in basic computer applications (Excel, Word, Outlook) is required.
• Ability to work collaboratively in a diverse team and manage challenging discussions with patients and families.
