Advanced Practice Pharmacist (Ambulatory Care) Clinical Pharmacist

Job Description The Clinical Pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Performs all duties in a manner which promotes team concept and reflects the Sisters of Providence mission and philosophy. Required Qualifications PharmD Pharmacy Doctorate Or Bachelor's Degree Pharmacy Upon hire: California Pharmacist Registered Upon hire: Basic Life Support (BLS) - where applicable Preferred Qualifications 1 year Acute care hospital pharmacy experience or equivalent experience. Completion of ASHP-accredited PGY1 residency.
