Plans, implements and evaluates nursing care for patients and families./ppEssential Functions/pulliProvides and directs comprehensive health care services to a caseload of patients of all ages and their families through direct care, supervision and interdisciplinary coordination./liliPerforms comprehensive initial and ongoing assessments to patients/families. Uses assessment skills to determine patients physical and mental status and status of environmental and social support./liliDevelops and modifies a plan of care for the patient and family based on a thorough assessment of problems, potential problems and the medical plan of treatment./liliResponsible for assessing, planning, implementing and evaluating physical end of life care for the patient in coordination with the attending physician and members of the Hospice Interdisciplinary Team./liliPlan nursing interventions that are consistent with the standard nursing practice./liliIdentifies and assesses patients/families health and social problems, potential problem areas, needs and resources of individuals, families and significant others./liliCoordinates Hospice and interdepartmental services, medical care, community resources and resources within the home in developing the plan of care./liliImplements services and completes medical records in accordance with the care plan in compliance with the the third party regulators, certified Home Health requirements, IMH Hospice policies, procedures, protocols, and standards of nursing practice and all Federal, State and local laws/regulations./liliTeaches and demonstrates care to the patient/family, utilizing established, available patient education, principles, guidelines, and resources; assesses effectiveness of patient/family teaching toward fostering independence in self-care./liliProvides supervision to ancillary health care workers, including but not limited to, Iroquois Hospice aides, assuring appropriate implementation and follow through on the Plan of Treatment./liliPrepares, administers and documents medications and treatments as prescribed./li/ulpEducation/Experience Requirements/pulliGraduated from an accredited school of nursing./liliOne year of experience./liliPositive attitude, good mental and physical health./li/ulpLicenses/Certifications/pulliCurrent state of Illinois RN/LPN license./liliCPR Certification./li/ulpBenefits/pulli401K/liliPaid Time Off/li/ulpIroquois Memorial Hospital is a 25 bed hospital located in Watseka, Illinois. The hospital facility is comprised of clinical and diagnostic services which are dedicated to providing high quality health care services to the community. These services include but are not limited to an Emergency Room, full service OR suites, an inpatient care unit, therapy services, advanced medical imaging, Home Health, and Hospice Care. The hospital operates rural health clinics in Watseka, IL, Gilman, IL, Milford, IL, Saint Anne, IL and Kentland, IN. The hospital also operates a skilled nursing facility, Iroquois Resident Home, which is attached to the hospital property./p/div
