Role Occupational Therapist responsible for delivering personalized, one-on-one care to older adults in their homes or senior living communities, utilizing the Geriatric House Calls™ model. Key Responsibilities Conduct evaluations, develop individualized care plans, and administer evidence-based therapy to enhance mobility, independence, and wellness. Document patient progress accurately within electronic medical records, ensuring compliance with regulatory standards. Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Utilize telehealth and virtual care to increase access and flexibility. Qualifications & Skills Valid Occupational Therapy license and degree from an accredited program. Proficiency with computers and EMR systems. Current CPR certification. Ability to perform physical tasks such as lifting, repositioning, and assisting with transfers.
