The Home Health Physical Therapist Assistant delivers specialized physical therapy support to patients in their homes, emphasizing mobility improvement, rehabilitation, and enhanced quality of life. Collaborating as part of a medium multidisciplinary team, this role requires daily travel within the service area and reports directly to a Physical Therapist. Responsibilities

• Conduct patient assessments to determine therapy needs
• Assist in developing and implementing individualized treatment plans
• Provide mobility training to promote patient independence
• Document patient progress and update care plans accurately

• Evaluate patient safety and recommend necessary equipment
• Perform follow-up visits to monitor and adjust treatments

Preferred Qualifications

• Minimum 1 year of experience as a physical therapy assistant
• Current state Physical Therapist Assistant license
• Strong skills in patient assessment and rehabilitation techniques
• Expertise in mobility training and therapeutic exercises
• Excellent communication and time management abilities
• Demonstrated empathy and patient-centered approach
