Position Summary We have an exciting opportunity to join our team as a Transplant Coordinator, Registered Nurse – Heart Transplant. In this role, the successful candidate will be an important member of the multidisciplinary Heart Failure/Transplant team. The HF RN will oversee care for advanced heart failure patients in the outpatient setting, collecting clinical information about patients, summarizing medical data, ensuring diagnostic testing and treatment procedures for patients are completed, triaging phone calls from patients, monitoring CardioMEMs Pulmonary Artery Pressure Monitoring system, providing education to patients and families about heart failure symptoms, management, and diet, and interfacing with multiple providers to ensure optimal care. Job Responsibilities Return HF triages via email, phone calls (urgent), and in‑basket messages. Provide excellent patient services to ensure satisfaction. Act as liaison with the inpatient team to ensure continuous care for the advanced heart failure patients. Communicate with the multidisciplinary team to optimize patient care, interfacing with physicians, social workers, nurse practitioners and others. Maintain contact with assigned caseload between visits based on their particular needs, initiating appropriate responses to issues and problems during these contacts. Ensure laboratory, radiologic, and diagnostic studies are scheduled (with FGP medical secretary) and completed. Educate patients on low‑salt/high‑K diet, heart failure symptoms, and management. Complete disability, home health care, FMLA paperwork with FGP medical secretary. Complete paperwork for prior authorizations and denials/appeals with FGP medical secretary. Prepare information for peer‑to‑peer authorizations. Keep a running list of patients who need follow‑up phone calls. Follow up on Heart Failure sign‑out. Obtain inpatient CardioMEMS readings. CardioMEMS monitoring: three times weekly transmission review, weekly notes, weekly phone call, coordinating tech support, reviewing abnormal values with NP, lab follow‑up, inpatient implant education, pre‑implant patient education. Perform simple stress testing. Follow patient labs and tests, alert NP if abnormal, call patient if results normal. Minimum Qualifications Current registration as a Professional Nurse in New York State. Bachelor’s Degree in Nursing. At least five years of clinical nursing practice, with the ability to apply the nursing process toward achievement of specific outcomes in an organized fashion. Self‑directed worker, able to work independently; excellent interpersonal, communication, and organizational skills; creative problem solving and critical thinking ability; understanding of complex antibody profiles. Previous experience in a heart failure patient population. CHFN certification desirable. Registered Nurse License – NYS. Salary Transparency The salary range for this role is $132,017.38 – $156,767.38 annually. Actual salaries depend on a variety of factors, including experience, specialty, education and hospital need.
