RN Case Manager

Mercy is seeking an RN Care Manager to coordinate comprehensive patient care across the continuum in our Healthcare & Medical Services department. In this role, you will perform clinical assessments, develop and implement care plans, facilitate transitions of care, and collaborate with interdisciplinary teams to optimize outcomes and reduce readmissions. You will educate patients and families, advocate for appropriate resources, and utilize evidence-based guidelines and EMR tools.
