Physician OB GYNE - ACCESS Evanston

Access Community Health Network

Position Summary Provide the full scope of services which fall under provider’s field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance, while following guidelines contained within the quality management, risk management, infection control, customer service and safety programs for Access Community Health Network. Loan repayment potential through National Health Service Corps (NHSC) NHSC LRP: Up to $75,000 Full Time for 2 years NHSC SUD-W LRP: Up to $75,000 Full Time for 3 years NHSC LRP or SUD-W LRP: $5,000 award enhancement for clinicians who demonstrate Spanish-language proficiency. Comprehensive benefits package 232 Hours of Paid Time Off (PTO) per year & Paid Holidays 40 Hours of CME & $2,000 Reimbursement Malpractice liability provided by the Federal Torts Claims Act (FTCA) Outpatient/Inpatient practice Supports H-1B visa and J1 Waivers Flexible schedules Clinical And Operational Responsibilities General Clinical Obtains health history and performs physical examinations. Diagnoses, treats, counsels, and coordinates care for each patient. Reviews and updates problem list and longitudinal plan of care at every visit. Meets documentation standards and completes all progress notes within 24 hours following the patient visit. Fulfills on-call responsibilities and responds to calls within 20 minutes. Prescribes medication in accordance with Illinois statute and professional practice guidelines. Performs all duties in conformity with quality standards and clinical guidelines established by ACCESS. Meets Meaningful Use standards, using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities. Manages EHR In-Basket, transfers responsibilities to colleagues when appropriate, and responds to messages within 72 hours. Patient Support and Education Provides culturally competent care. Recognizes patients’ families as an integral part of the care team. Ensures patients’ and families’ needs are met and clinic policy is followed. Encourages patients and families to voice questions and/or concerns. Works with patients and their families to encourage care self-management. Conducts patient education activities, including pre and post instruction exams and procedures, and reinforcement of follow-up instructions. Utilizes printed health education materials and documents all instructions given. Works with care team and case management to provide coordinated delivery of case management services. Ensures patients presenting with greater acuity than clinically appropriate are transferred appropriately. Meets intensity of service and/or severity of illness admission criteria. PCMH Requirements Coordinating with health center staff to ensure patient needs are met. Participating in health center huddles and meeting scheduled health center times and productivity standards. Ensuring patients are seen in a timely and clinically appropriate manner and involving health center employees in the process. Ensuring care teams properly carry out effective treatment, education and perform clinical tasks in accordance with policy and procedure. Provider Development Participating in orientation and training of staff as needed. Ensuring care team meets internal certification requirements for patient education and medication administration. Participating in proficiency testing as required by ACCESS. Possibly functioning as a preceptor in teaching programs. Adhering to HRSA and FTCA regulations. Other duties as assigned. Requirements Doctor Osteopathy (D.O.) or Doctor of Medicine (M.D.) required. Completed residency in primary care specialty and Board Certification required. Minimum two (2) years of experience in primary care medicine in an outpatient setting preferred. Current licensure in the State of Illinois, DEA, Controlled Substance and CME requirements. Knowledge of applicable rules, regulations and standards related to quality assurance, safety and infection control required. Electronic Health Records competency; EPIC knowledge preferred. Competencies/Behaviors Cultural competency. Interpersonal skills. Administrative and organizational skills. Working Conditions/Equipment Clinical/Health Center environment. Ability to concentrate and communicate with frequent interruptions. Ability to stand, sit and walk for up to eight hours within Health Center. Ability to lean and stretch in patient exam context. Computer/laptop. Phone/Fax/Copier/Scanner. Additional Information Access is a network of federally qualified health centers serving the community. Candidates may be required to be vaccinated against communicable diseases and provide supporting documentation, or apply for religious and/or medical vaccination exemption as part of the application process. The pay ranges represent the minimum to mid-range for positions, determined based on experience, education, market conditions, and grant funding. We are an equal opportunity employer.
