Job Title Home Health Speech Language Pathologist Location Plainfield, IL, USA (servicing Plainfield, Aurora, Naperville and surrounding areas) Company Residential Home Health and Hospice (‘Residential’) Responsibilities Collaborate with the care team to manage and deliver patient care plans for home health speech language services. Provide patient care to individuals in Plainfield, Aurora, Naperville and surrounding areas. Consistently meet critical charting deadlines and maintain accurate clinical documentation. Use tablets, mobile phones and EMR software for assessment, treatment planning and charting. Maintain a valid driver’s license and commute to patient locations as required (~20% of time). Qualifications Master’s degree from an accredited speech and hearing program with certification by the American Speech and Hearing Association. License and certification as a Speech Therapist issued by the state in which you will work. Minimum of one year of healthcare experience. Strong time‑management and self‑management skills, capable of meeting critical deadlines. Experience with tablets, mobile phones, and EMR software. Drug‑screening and background check clearance before employment. Compensation & Benefits Competitive pay per visit compensation model, with potential for upward earning.
