You’ll work independently in the field while being fully supported by our Clinical Managers, QA team, and administrative staff.Key ResponsibilitiesPerform comprehensive occupational therapy evaluations, including SOC, Recertification, and Discharge assessmentsDevelop and implement individualized treatment plans focused on functional independence and safetyProvide interventions including ADL/IADL training, upper extremity strengthening, fine motor coordination, cognitive strategies, and adaptive equipment recommendationsEducate patients and caregivers on home safety, energy conservation, and compensatory techniquesAccurately complete all documentation in the EMR within required timeframesCollaborate with physicians, nurses, therapists, and interdisciplinary team membersParticipate in case conferences and care coordination as neededMaintain compliance with Medicare, ACHC, and agency standardsCurrent FL licensure as an Occupational Therapist (OT).Minimum 1 year of clinical experience; home health experience preferred.Familiarity with WellSky EMR or similar documentation systems preferred.Strong clinical reasoning, time management, and communication skills.Ability to work independently and manage a flexible field-based schedule.Reliable transportation, valid driver’s license, and current auto insurance.Work Location: On the road #J-18808-Ljbffr
