Pace Social Work Supervisor Job Overview The PACE Social Work Supervisor position is responsible for administrative oversight and supervision of direct reports at the assigned PACE site, which will include the master level social workers and other PACE team members as needed. The role promotes effective collaboration and individualized care planning among interdisciplinary team members. Responsibilities Coordinate routine assessments and care plans. Assist with family conferences and community referrals. Monitor team member documentation. Onboard and orient new hires. Conduct routine competency and work performance evaluations. Provide guidance and support related to participants’ behavioral health concerns and advanced life planning/end‑of‑life care. Ensure compliance with state and federal regulatory requirements related to social work department tasks. Collaborate with other site leaders to monitor site safety, overall staffing changes, and program budgets. Provide coverage of SW department roles as needed. Minimum Requirements Master's degree in social work (MSW) from an accredited university. LCSW preferred. Minimum 2 years’ experience in a supervisory position in a health‑related or human services field. Minimum 2 years of experience in case management, social advocacy, or care planning with the geriatric population preferred. Bilingual Spanish/English, Chinese/English, or another second language strongly preferred. Valid BLS certification or higher, following AHA or American Red Cross guidelines. Compensation $90,090.88 - $112,613.60 annually (range subject to change). This role may also qualify for discretionary bonuses or incentives.
