Physical Therapist - Hospital Rehab - Full Time

Job Title: Physical Therapist – Hospital Rehab Location: Elliot Hospital, 1 Elliot Way, Manchester, NH Hours: 40 hours/week; shift for 4-10 or 5-8 workdays. Start time flexible 7:30 AM-9:30 AM. Weekend rotation every 5th weekend. Rotating holidays required. Sign-On Bonus Eligible up to $10,000. Responsibilities Prepare for patient evaluation by reviewing medical records to identify focus areas, contraindications, and precautions. Ensure therapy orders are active and appropriate. Evaluate the skilled therapy needs of the referred patient and develop a written, prioritized plan of care with short‑and‑long‑term goals in collaboration with the patient and the health‑care team. Update treatment plans and re‑evaluate patients, communicating with referrers and other clinicians involved in care. Establish and implement discharge plans, instructing patients, family members, and essential personnel on effective home exercise programming or other strategies to maximize function. Provide clinical supervision of therapy assistants, rehabilitation assistants, and other essential personnel in the implementation and revision of the rehabilitation plan of care. Participate in EHS Rehabilitation Services’ clinical development program, including delivering an educational presentation once per year on a chosen topic. Collaborate with interdisciplinary teams (nursing, case management, providers, and ancillary services) across ICU, CICU, Pediatrics, Medical Surgical, and the ED. Support early mobility and functional return goals toward optimal discharge outcomes. Qualifications Graduate of an accredited Physical Therapy Program with an Entry Level Degree. Licensed as a Physical Therapist by the State of New Hampshire Physical Therapy Governing Board. CPR certification required or able to complete within the first 90 days.
