Role Description This is a full-time, on-site Home Health Licensed Practical Nurse (LPN) role based in South Chesterfield, VA. The Home Health LPN will provide direct patient care for an amazing 10-year old child with a G-Tube and tracheostomy. Care to be provided in clients’ home, and includes accompanying the client to school to provide care throughout the day during the school year. Duties include monitoring vital signs, administering medications and treatments as ordered, and assisting with activities of daily living. The role involves implementing care plans and documenting patient status accurately. Nurse is needed Monday - Friday, 7:00 am - 4:00 pm. Qualifications Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Virginia, with solid foundational Nursing and Medicine knowledge. Demonstrated experience in Home Care and in-home Pediatric Patient Care, including monitoring, medication administration, and support with daily living activities. Strong clinical assessment, documentation, and time-management skills. Excellent interpersonal and communication skills to work effectively with the patient, family, caregivers, and interdisciplinary team members. Current BLS/CPR certification, valid driver’s license, reliable transportation for home visits, and ability to meet physical demands of home health work. Familiarity with electronic health records (EHR) and adherence to HIPAA and infection control standards; prior home health or community health experience is preferred. #J-18808-Ljbffr
