Overview Department: GCH Social Work Location: Golisano Children's Hospital Location of Job: US:NY:Buffalo Work Type: Part-Time Benefit Eligible Scheduled Work Hours: 8a-4p, 8:30a-4:30p, 9a-5p Responsibilities Assess psychosocial needs of patients, families, and significant others. Provide professional counseling, case management, and discharge planning services in collaboration with the multidisciplinary health care team to support appropriate use of medical care and optimal functioning. Assist patients and families in navigating transitions across care settings in the community and alternate levels of care. Education and Credentials Master of Social Work (MSW) or related field required. NYS Driver's License required on hire. For School Health Services at OCH, license must be active and maintained. PASRR Screen within 6 months required. LMSW, LCSW, or LCSWR required for Oishei Children's Hospital Psychiatry Clinic and School Health Services at OCH (must be active and maintained). NYS DOH PASRR screen certification is required (regulatory) within six months of hire/transfer for all inpatient departments. If not offered in 6 months, must take the next available class. Basic Life Support (BLS) required for all school-based clinics within 60 days of employment. Existing MSWs as of 9/24/19 have until 11/24/19 to obtain BLS and must maintain thereafter. Experience Three years of experience in discharge planning in an acute care or nursing home setting for inpatient departments, including transitions from acute care or nursing home settings and educating patients and families on placement and community services. Three years of experience in counseling is required, excluding School Health Services departments. Inpatient departments: three years of discharge planning experience in an acute care or nursing home setting; nursing home knowledge preferred. School Health Services only: one year of experience in child and adolescent mental health counseling with school-aged children and related mental health concerns.
