- Conduct assessments of patients in both inpatient and outpatient environments and craft tailored treatment plans
- Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
- Work collaboratively with nurses, physicians, case managers, and fellow rehabilitation experts to optimize patient outcomes
- Inform patients and their families about therapeutic objectives, discharge processes, and home exercise regimens
- Maintain documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
- Engage in care team meetings and interdisciplinary discussions to ensure seamless care transitions
- Doctorate in Physical Therapy (DPT) or a comparable qualification from an accredited institution
- State licensure as a Physical Therapist
- CPR certification is preferred
- Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory
- Previous relevant experience strongly preferred
- Adept at managing patients with complex medical and mobility challenges
- Excellent communication and teamwork capabilities
- Familiarity with digital charting and clinical documentation systems
