Responsibilities
- Provide direct patient care under direction of the supervising RN, including monitoring and documenting vital signs.
- Administer prescribed medications and treatments per physician orders and facility policy.
- Assist residents with activities of daily living and mobility, promoting comfort and safety.
- Monitor for changes in residents' condition and promptly report to the supervising nurse.
- Document care accurately in the electronic health record and participate in care planning with the interdisciplinary team.

Requirements
- Active Licensed Practical Nurse license in New Hampshire (in good standing)
- Graduate of an accredited practical nursing program
- Current CPR/BLS certification
- Prior experience in long-term care or skilled nursing facility preferred
- Ability to work evening shifts and meet assignment requirements, with competency in documenting care
