Nurse Practitioner (NP) Medical Director Services PC is looking to hire experienced Nurse Practitioners (NP) to service our skilled nursing facility located in West Caldwell, NJ. Excellent salary will be offered, commensurate with experience. Responsibilities Serves as primary care provider to long- and short-term residents. Assessing patient health by visiting patients; performing physical examinations; obtaining, updating, and studying medical histories. Team-based care: collaborating with the medical attending/physician and nursing home staff to provide the highest standard of care attainable. Build relationships with residents and families; educate and formulate a plan of evidence-based care aligned with the patients’ needs and desires. Complies with federal, state, and local legal and professional requirements by studying legislation, enforcing adherence, and advising management on needed actions. Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. Maintain a safe and clean working environment by complying with procedures, rules, and regulations; ensure standards align with the organization\'s core values of being respectful, professional, and cognizant of all cultural, personal, and religious beliefs. Review and ensure charting, documentation, and administration of care are up to date and performed in a timely and efficient manner. Location West Caldwell, NJ About Us Medical Director Services was founded in 2016 when SNFs were shifting a focus on value while treating residents with higher acuity. MDS PC emphasizes quality and customer service first. A crucial focus is treating in place when possible and reducing unnecessary hospitalizations. MDS PC is experienced in providing medical services in an SNF setting with an understanding of how to deliver effective medical care in an SNF and an understanding of the survey process. We work with facilities with specialty units and provide appropriate medical personnel with experience in treating residents on vents. It is standard for our providers to reach out to surrounding hospitals to ensure good communication for continuation of care. We focus on reducing the number of medications as well as narcotics and psychotropic meds; if providers determine a medication is appropriate, we ensure it is well documented so the facility is not cited. #J-18808-Ljbffr
