- Assess patients in both inpatient and outpatient environments and develop tailored treatment plans.
- Deliver physical therapy interventions with a focus on enhancing mobility, strength, balance, and overall function.
- Work collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to optimize patient outcomes.
- Provide education to patients and families about therapeutic goals, discharge planning, and at-home exercises.
- Maintain detailed documentation of evaluations, treatments, and patient progress following facility protocols and regulatory standards.
- Engage in multidisciplinary care team discussions and conferences to ensure cohesive care delivery.
- Doctorate in Physical Therapy (DPT) or an equivalent accredited degree.
- State licensure as a Physical Therapist .
- CPR certification preferred.
- Specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory.
- Experience is strongly favored.
