RN - Case Manager

Registered Nurse (RN)We are seeking a dedicated Registered Nurse (RN) to join our team in a case management role. The ideal candidate will have significant experience in acute and ancillary care across multiple modalities and a passion for supporting patients through their healthcare journey.Key ResponsibilitiesConduct comprehensive assessments for case management.Collaborate with healthcare professionals to develop and implement care plans.Coach and guide patients/members through their healthcare journey.Monitor patient progress and adjust care plans as necessary.Engage in utilization management activities to ensure efficient care delivery.QualificationsEducation: Bachelor's Degree in Nursing (BSN) preferred.Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required.Certified Case Manager (CCM): Preferred.Experience: 5-10 years of nursing experience, particularly in acute and ancillary settings.Experience in utilization management and case management in a health plan is preferred.Skills: Strong communication skills and demonstrated ability to coach patients/members.Additional InformationThis position is full-time, operating Monday through Friday, during day shift hours.
