What Role Will You Play? Social Services Clinical Case Managers are part of an interdisciplinary treatment team charged with providing quality clinical case‑management services to patients from admission to discharge. They assess patient needs, collaborate with patients to create safe and appropriate discharge plans, participate in diagnostics and treatment planning, and work with patients’ families, support staff, and outpatient teams for coordination of the discharge plan. What Will You Get To Do? Conduct psychosocial assessments to document patient history and identify preliminary barriers to discharge. Perform diagnostic discharge planning and case management starting from the day of admission. Collaborate with outpatient providers and support staff to ensure a successful transition. Identify and recommend family and/or community resources such as housing, treatment providers, and other recognized resources. Participate in interdisciplinary treatment team meetings as a core professional and clinical member. Act as liaison for hospitalized patients with various community agencies and resources. Document clinical case management in the medical record. Facilitate therapeutic meetings between patients and outside supports to coordinate discharge planning. Qualifications A bachelor’s or master’s degree from an accredited college or university in a mental or behavioral health field is required (an MSW is preferred), along with a desire to help patients improve their lives. Candidates must obtain an Agency‑Affiliated Counselor credential unless they have a clinical license. All Fairfax employees must successfully complete the Handle With Care course during New Employee Orientation. A positive attitude and strong organizational skills are essential, as is the ability to meet regulatory deadlines and pivot based on competing priorities.
