General Purpose The primary responsibility of this position is to supervise the day‑to‑day nursing activities of the facility during your tour of duty. Supervision must be conducted in accordance with current federal, state, and local standards, guidelines and regulations governing the facility and may be required by the Director of Nursing (DON) and/or Assistant Director of Nursing (ADON) to ensure the highest degree of quality care is maintained at all times. Administrative Functions Assist the Director of Nursing Services in directing the day‑to‑day functions of nursing activities. Develop, maintain, and update written policies and procedures governing the nursing service department. Ensure nursing station reference material (e.g., PDRs, regulations, professional standards) is current and recommend additional material as needed. Maintain the Nursing Service Procedures Manual and enforce its compliance. Prepare written and oral reports to the Director regarding the operation of the nursing service department. Review and recommend revisions to department policies, procedure manuals, job descriptions, etc. Coordinate nursing services with other resident services to ensure continuity of care. Ensure compliance with job descriptions and quality assurance program participation. Assist with facility surveys and inspections requested by government agencies. Review and update resident written discharge plans and assist with nursing service plans for discharge. Meet with nursing staff and support personnel to plan services, programs, and activities. Admit, transfer, and discharge residents as required. Complete accident/incident reports and perform necessary administrative duties (e.g., medical forms, evaluations). Protect user ID, passwords, and patient protected health information and report any violations. Report unauthorized attempts to access the facility’s information system. Committee Functions Attend required committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment). Prepare written and oral reports for committees as directed. Provide information to the Quality Assurance and Assessment Committee. Assist in developing the Care Plan Committee agenda and review discharge plans. Implement committee recommendations under the Director’s guidance. Personnel Functions Ensure sufficient licensed practical and/or registered nurses and certified nursing assistants are available during each tour of duty. Develop work assignments, delegate authority to Charge Nurses, and ensure training compliance for nursing assistants and trainees. Conduct daily rounds to verify proper performance of nursing standards and report findings to the Director. Monitor absenteeism, enforce dress codes, review complaints, grievances, and maintain workplace rapport. Conduct employee performance evaluations, determine staffing requirements, and assist with personnel decisions. Enforce discipline fairly and report occupational exposures, fraud incidents, and security breaches. Nursing Care Functions Inform staff of new admissions and ensure rooms are prepared. Greet and escort newly admitted residents to their rooms. Coordinate physician visits, encourage timely documentation of orders, and report delays. Collaborate with residents, families, and physicians on care planning. Conduct daily resident evaluations and ensure direct care is provided by qualified staff. Review nurse notes for completeness and compliance with resident wishes. Perform direct nursing care, including IV start, specimen collection, and vital sign monitoring. Assist Charge Nurses in monitoring seriously ill residents and authorize restraints per policy. Ensure medication administration, storage, and treatment schedules comply with regulations. Inspect treatment areas, check residents unable to call for help, and arrange transportation for discharge as needed. Inform families of resident deaths and coordinate funeral arrangements. Staff Development Plan, conduct, and schedule in‑service training classes on topics such as OSHA, TB, HIPAA, Abuse Prevention, Safety, and Infection Control. Assist in planning clinical supervision for nurse aide trainees. Attend continuing education programs to maintain licensure and stay current with professional developments. Safety and Sanitation Assist the Safety Officer in developing safety standards for the nursing service department. Identify and record tasks involving potential exposure to blood/body fluids and provide required training. Maintain personal protective equipment supplies and enforce proper use and disposal. Ensure hand hygiene procedures are followed and participate in fire safety and disaster preparedness drills. Support the infection control program for monitoring communicable diseases and report hazardous conditions. Ensure safe patient handling and the use of safety equipment during lifting or moving residents. Equipment and Supply Functions Recommend equipment and supply needs to the Director. Maintain adequate stock levels of medications, supplies, and equipment. Assist the Maintenance Director with preventive maintenance programs for nursing equipment. Ensure safe operation of equipment and that only trained personnel use it. Monitor efficient use of supplies to reduce waste and maintain MSDSs for hazardous chemicals. Care Plan and Assessment Functions Develop written preliminary and comprehensive assessments of each resident’s nursing needs. Develop comprehensive care plans identifying problems, goals, approaches, and responsible professionals. Encourage resident participation in care plan development and review. Review care plans for appropriateness and ensure staff reference them during care. Assist the Resident Assessment/Care Plan Coordinator with MDS planning, RAPs, and triggers. Verify that treatments align with the resident’s care plan and wishes. Budget and Planning Functions Monitor economic conditions and recommend adjustments in nursing services to maintain quality care. Report fraudulent activity related to billing, cost reports, or kickbacks. Supervisory Requirements Provide overall supervision of and management of nursing staff, delegating administrative authority and accountability to fulfill assigned duties as an RN. Qualifications Must possess a Nursing Degree from an accredited college or university. Experience: 2 years or more in a long‑term care facility, with training in rehabilitative and restorative nursing practices. English proficiency: Read, write, speak, and understand the English language; read technical procedures and policy manuals; present information effectively. Mathematical skills: Apply fractions, percentages, ratios, and proportions to practical situations. Reasoning and leadership: Make independent decisions, deal tactfully with personnel and the public, supervise effectively, and implement policies and procedures to provide quality care. Professional knowledge: Nursing and medical practices, laws, regulations, and guidelines specific to nursing care facilities; patience, tact, and enthusiasm. Willingness to pursue new methods and incorporate them into nursing practices. Certificates, Licenses, Registrations Current, unencumbered, active RN license in the state. Current CPR certification. Knowledge and experience with PCC preferred. Maintain continuing education/licensing requirements and remain in good standing with the State Board of Nursing. Physical Demands Move intermittently throughout the workday. Speak and write English clearly. Manage mental and emotional stress. Use prosthetics as required to meet sensory functions. Perform tasks requiring 25 pounds to 5 feet and 50 feet distance; assist in emergency evacuations if necessary. Work Environment The role involves working in office areas and throughout the nursing service area, including drug rooms, nurses’ stations, and resident rooms. Work includes frequent interruptions, interaction with residents, personnel, visitors, and government agencies. The position requires evening, weekend, and holiday work, and may involve call‑backs during emergencies. Exposure to falls, burns, dust, disinfectants, smoke, hazardous chemicals, infectious waste, TB, HIV, hepatitis B, and other infective substances is possible.
