You support residents through: Assessments and contributions to individualized care plans Medication administration and treatment management Documentation and clinical communication Monitoring changes in condition and collaborating with care partners Ensuring residents maintain dignity, independence, and quality of life Approaching every interaction with patience, professionalism, and a warm smile This role is ideal for an LPN who values teamwork, person-centered care, and the opportunity to build meaningful, long-term relationships with residents and families.
