This on-site position is essential in overseeing the quality of care provided by our field staff, ensuring coordination with physicians and clinicians to deliver the best possible outcomes for our patients.

Position Overview

Responsibilities:

• Care Oversight: Monitor and ensure case management guidelines are being followed by each team member. Provide employees with continuous training and education and performing employee reviews and meetings. Communicate with leadership regarding continuous quality improvement, problem solving, and issues related to overall effectiveness of the Home Health services.

• Coordination: Service as the first line for resolution of concerns and issues. Collaborate closely with physicians, clinicians, and other healthcare professionals to develop and implement patient care plans.

• Patient Advocacy: Serve as a liaison between patients, families, and healthcare providers to advocate for the patient’s needs and preferences.

• Documentation: Coordinate with QA team to maintain accurate and timely records of patient care, including assessments and care plans.

• Compliance: Ensure all care provided meets state and federal regulations as well as company policies.

Qualifications

• Experience: Minimum of 2 years of experience as a Home Health RN.

• EMR Proficiency: Experience using WellSky (Kinnser) EMR for home health is preferred.

• Licensure: Current RN license in the state of Illinois.

• Skills: Strong organizational, communication, and problem-solving skills. Ability to work independently while managing multiple priorities.

• Team Player: Ability to work collaboratively with a multidisciplinary team.

Benefits:

Pay: $35.00 - $40.00 per hour

Expected Hours: 40 per week (weekend as needed)

Experience Nursing: 2 year (Required)

License / Certification: RN (Required)
