Overview

Responsibilities
- Direct and coordinate an activity program to meet patients’ social, recreational, physical, educational, and spiritual needs with meaningful activities.
- Assess patient/resident needs and develop a comprehensive activity plan aligned with care goals; monitor and adjust programs for engagement and dignity.
- Oversee program development, performance improvement, and budget monitoring; supervise staff and provide training as needed.
- Communicate effectively with patients, families, and interdisciplinary team members; ensure coverage across shifts, including evenings, weekends, and holidays when required.
- Maintain documentation of activities, outcomes, and participation; ensure compliance with regulatory and safety standards.
- Collaborate with other care team members to integrate activity plans with overall treatment and discharge goals; participate in quality improvement efforts.

Requirements
- Three (3) years of work experience as an Activities Coordinator with planning, assessing, and coordinating a comprehensive activity program.
- Must have Minimum Data Set (MDS) experience.
- Basic Life Support (BLS) for Healthcare Workers certification.
- Valid driver’s license.
- Ability to read, write, speak, and understand English; strong communication and supervisory skills.
- Willingness to work on shifts including evenings, weekends, and holidays as needed.
