Wellness Home Services Licensed Practical Nurse (LPN)

Role Overview In this role, you will be responsible for delivering individualized, one-on-one care tailored to each resident’s unique needs, under the direction of a Registered Nurse (RN) where required by state guidelines. You will specialize in managing chronic conditions, supporting post-operative recovery, and addressing complex medical needs with a steadfast commitment to safety, dignity, and comfort. You will perform medication management, vital sign monitoring, collaborate with multidisciplinary care teams, and provide education and guidance to residents and families. You will be committed to delivering exceptional, high-quality care that improves health outcomes, promotes independence, and enhances overall quality of life. Responsibilities Deliver individualized care to residents in a home setting within the guidelines of state regulations and nursing protocols. Qualifications Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing Minimum of 1–2 years of clinical nursing experience, preferably in acute care or home health setting Current CPR certification (or obtain within three months of employment) If responsible for transporting a resident, maintain a valid state driver’s license and provide proof of current automobile liability insurance Benefits Team members are eligible for a generous benefit package including health benefits (medical, prescription, dental, and vision), flexible spending accounts, life insurance, disability programs, a 401(k) plan (with 4% company match after one year of employment), paid time off and holidays, and additional benefits. Eligibility may vary based on status. Pay Range $24.38 – $36.86 per hour. Starting rate will vary based on skills and experience.
