Nurse Practitioner or Physician Assistant - Primary Care Provider - Braintree

Braintree, MA | Atrius Health Atrius Health, part of the Optum family, is seeking an experienced Nurse Practitioner or Physician Assistant to join its primary care team in Braintree. This role offers the opportunity to independently manage a patient panel while working within a collaborative, value-based care model focused on quality outcomes and whole-person care. Position Highlights Independently manage a panel of primary care patients Provide comprehensive preventive, acute, and chronic disease care Collaborative, team-based practice environment Partner with RNs, pharmacists, behavioral health, specialists, and population health teams Focus on continuity of care, care coordination, and patient outcomes Practice within a nationally recognized, physician-led organization Strong clinical support and opportunities for professional growth Required Qualifications Nurse Practitioner (NP): Master's degree (MSN) and national certification (ANCC or AANP) or Physician Assistant (PA): Master's degree and certification by NCCPA Active, unrestricted Massachusetts NP or PA license (or ability to obtain prior to start) Active DEA and MCSR (or ability to obtain prior to start) Current BLS certification Experience & Scope Minimum of three (3) years of experience independently managing a full primary care patient panel Experience includes responsibility for: Preventive care Longitudinal care Chronic disease management Care coordination and follow-up Experience caring for adult patients ages 65 and older Demonstrated experience practicing in an ambulatory primary care setting, not limited to episodic, urgent, or specialty-only care Prior responsibility for inbox management, test results follow-up, patient messaging, and referral coordination The salary range for this role is $165,000 to $190,000 annually based on full-time employment.
