Adult Reconstructive and Total Joint Orthopaedic Surgeon Needed near Jacksonville, Florida!!\n
\nPosition is available due to growth!\n
\nJoining a Multi-Specialty Group witih 8 Physicians / 6 Mid-Levels\n
\nApprox 8-9 Call weeks per year at the Hospital\n
\nOffice Hours are: 8am - 5pm\n
\nPractice:  Orthopaedic specialties include Sports Medicine;\n
\nJoint Replacement; Elbow & Shoulder; Foot & Ankle; Neck & Back and Hand & Wrist.\n
\nTwo locations within the County.  Both locations include:\n
\ndigital x-ray and MRI, orthopaedic rehabilitation and DME. The main location\n
\nincludes a state of the art 9,000 square foot outpatient surgery center.\n
\nCandidates must have:\n
\nAdult Reconstruction Fellowship / Board Certification / Florida State License\n
\nPatient Population:  Commercial 50%; Medicare 40%; WC 5%; Other 5%\n
\nAnticipated Patient Volume:  Approx 50 + patients per clinic day / three clinic days per week\n
\nSurgical cases at ASC one day per week + Hospital one day per week\n
\nSalary will be competitive - TBD\n
\nBenefits Include:  Paid Malpractice, CME ($2500 + 10 days per contract year), Vacation (15 days of PTO each contract year)\n

\n*Partnership:  Buy-in opportunity at the end of year two as equal partner.\n
\nAdditional Information - Bonus Structure:\n
\n - First Contract Year. Physician shall be entitled to twenty five percent (25%) of the\n
\nNet Collections in the First Contract Year that are over two times the First Contract\n
\nYear Base Salary.\n
\n - Second Contract Year. Physician shall be entitled to thirty three percent (33%) of\n
\nthe Net Collections in the Second Contract Year that are over two times the Second\n
\nContract Year Base Salary.\n
\n - Notwithstanding the foregoing, in the event Net Collections exceed two times the\n
\nContract Year Base Salary plus One Hundred Thousand and 00/100 Dollars\n
\n($100,000) for each year of the contract (for example. over $500,000 for the First\n
\nContract Year) (the Additional Bonus Base), Physician\n
\n
