About the Job Speech Therapist (ST) - Home Health (Full Time or Part Time) Location: Alameda, CA Position Overview Work as a Speech Therapist in our Home Health team, providing assessment, treatment planning, and ongoing management of patients' speech and communication needs across the Alameda area. Collaborate with the Clinical Director and other healthcare professionals while working independently in the field and remotely. Responsibilities Provide, coordinate, and oversee skilled speech therapy services per the patient’s individualized plan of care, in collaboration with the Interdisciplinary Team. Conduct initial and ongoing evaluations of patient status. Develop and implement treatment plans, monitor patient progress, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing. Deliver educational information to patients and their families regarding speech and language disorders and treatment plans. Maintain accurate clinical records, submitting documentation in a timely manner per company policy. Adhere to all company policies and procedures to ensure the highest standards of care. Requirements Current OT License to practice in California. Experience in Home Health or Hospice care - 1 year required. Current CA Driver’s License.
