Emblem Home Health is seeking a dedicated RN to manage patient intake, referrals, and admissions for home health. You will verify insurance, coordinate services, and ensure a smooth transition of care while building strong relationships with physicians and hospitals. The role requires an AZ RN license, patient intake or care coordination experience, and CPR certification.
