Licensed Practical Nurse (LPN) – Night Shift Opening Join the Rolling Meadows Family & make a real difference caring for residents in a warm, family‑like environment at Rolling Meadows Health & Rehabilitation Center in La Fontaine, Indiana. Your Role as an LPN Provide high‑quality skilled nursing care aimed at restoring residents to their highest potential. Support individualized care plans across skilled nursing, memory care, and rehabilitation neighborhoods. Work collaboratively with a dedicated interdisciplinary team known for professionalism, compassion, and outstanding resident relationships. Foster meaningful connections with residents, families, and coworkers in an environment focused on dignity and respect. Supervise, perform, and document care assignments and routine procedures to ensure the quality of care. Participate in the development and implementation of resident care plans, maintaining up‑to‑date records and making recommendations for revisions. Observe for symptoms, medication reactions, and treatment responses, obtaining vital signs, intake/output, weights, diet supplements, and cultures. Assist with the orientation and ongoing training of staff to assure required skill levels are maintained. Qualifications Graduate of an accredited school of nursing with a valid LPN license in good standing. Thorough understanding of safe and effective nursing practices within the State of Indiana. Basic computer skills, including knowledge of Excel, Word, and office machines such as fax and copying machines. Personal integrity and the ability to deal tactfully with residents, family members, visitors, government agencies, and other personnel. Passion for caring for people. Ability to maintain confidentiality of sensitive information and comply with Corporate Compliance and HIPAA. Discrete behavior and respect for the privacy of residents and staff.
