Work Environment

This role offers the flexibility of a remote work setting, allowing you to serve clients from the comfort of your home while maintaining a structured schedule. The work hours are set from 9 AM to 6 PM, Monday to Friday, ensuring a healthy work-life balance without evening or weekend commitments. Collaboration tools and communication platforms will facilitate daily interactions with colleagues and team members to ensure cohesive care delivery and professional support.

Key Responsibilities

• Conduct thorough assessments of mental health needs and develop individualized treatment plans that address the diverse challenges faced by clients.
• Implement and coordinate behavioral health services, ensuring adherence to best practices and emergency protocols.
• Engage as an integral member of the primary care team, fostering strong inter-professional relationships with healthcare providers and support staff to promote comprehensive care.
• Evaluate patients for crisis intervention needs and provide timely counseling to assist clients in managing immediate challenges.
• Perform intake assessments, documenting patient history and presenting issues accurately and comprehensively.
• Screen clients for substance abuse issues, including prescription drug, illicit drug, and alcohol use, providing referrals to treatment programs as necessary.
• Maintain compliance with mental health policies and procedures while ensuring the highest standard of care.
• Facilitate patient access to medication-assisted treatment (MAT) services and monitor progress, adjusting care plans as needed.

Patient Population / Client Focus

The population served by the Vitalize Metal and Wellness Center consists of a diverse array of clients, including individuals experiencing anxiety, depression, trauma, and relational issues. You will be tasked with addressing the unique needs and backgrounds of each patient, delivering culturally competent care that empowers them to take charge of their mental health journeys. Your ability to establish rapport and build trust with clients will be central to facilitating effective treatment.

Qualifications

• A Master's degree in Social Work or Counseling from an accredited institution.
• A current, valid license to practice as an LCSW in the state of New Jersey.
• Demonstrated proficiency in clinical skills, including crisis intervention and evidence-based therapeutic techniques.
• Exceptional communication abilities, both orally and in writing, facilitating effective interactions with clients and multidisciplinary team members.
• Strong commitment to teamwork, with the ability to collaborate across disciplines to provide comprehensive care.
• Proficiency with digital patient management systems and the ability to maintain accurate medical records.
• Awareness and sensitivity to working with culturally and ethnically diverse populations.

Preferred Experience

While all qualified applicants will be considered, candidates with prior experience in hospice or home healthcare settings will be preferred. Experience with telehealth platforms and remote patient counseling techniques will enhance your applicability, as these skills are vital for our remote work structure.

Compensation and Benefits
