Pharmacy Services

Provide comprehensive clinical pharmacy services to optimize medication therapy, patient outcomes, formulary compliance, and resource utilization.

Review and evaluate medication regimens, identify drug-related problems, adverse drug reactions, interactions, and recommend evidence-based therapeutic alternatives.

Perform medication reconciliation, obtain medication histories, document findings, and counsel patients on the safe and effective use of medications.

Develop individualized dosing regimens using pharmacokinetic principles, laboratory data, and patient-specific clinical factors.

Provide drug information, pharmacotherapy consultation, and clinical recommendations to physicians, nurses, and other healthcare professionals.

Evaluate non-formulary medication requests and recommend appropriate formulary alternatives when indicated.

Verify medication orders for accuracy and appropriateness, dispense medications, and ensure compliance with federal regulations and VA policies, including controlled substances management.

Perform quality assurance activities, monitor medication use, identify prescribing trends, and document clinical interventions.

Supervise, guide, and provide technical oversight to pharmacy technicians to ensure safe, accurate, and efficient pharmacy operations.

Participate in interdisciplinary committees, collect and report pharmacy performance data, and recommend policy or process improvements.

Train, mentor, and precept pharmacists, pharmacy technicians, students, externs, and residents while supporting continuing education and professional development.

Serve as pharmacist-in-charge when assigned by coordinating workflow, resolving operational issues, and ensuring continuity of pharmacy services.

Maintain compliance with HIPAA, information security, controlled substance accountability, and VA regulatory requirements.

Work Schedule: Monday - Friday 0930 to 1800, Rotating Saturdays 0800 to 1630
