About This Role The Care Navigator at Centene develops, assesses, and coordinates care management activities based on member needs to provide quality, cost-effective healthcare outcomes. This role is ideal for individuals with experience in case management and a background in health services. Highlights Location: Remote, MO Brand/Division: Centene Schedule: Full Time, Day shift Residency: Must reside in MO Work Style: Fully remote Pay: $22.94 - $38.79 per hour What You'll Do Evaluate the needs of the member, barriers to care, and recommend a plan for the best outcome. Develop or contribute to personalized care plans/service plans and identify needed providers and resources. Provide psychosocial and resource support to members/caregivers to access local services. Coordinate between the member and care provider team to ensure timely access to services. Monitor progress towards care plan goals and collaborate with healthcare providers for revisions. Collect, document, and maintain member information to ensure compliance with regulations. Perform on-site visits to assess member needs and collaborate with providers. Educate care managers and members on procedures, healthcare instructions, and benefits. Complete other duties as assigned by the people leader. Requirements Education: Bachelor’s degree required; Graduate from an Accredited School of Nursing if holding clinical licensure. Experience: 2 – 4 years of related experience required. For Sunflower Foster Care: Bachelor’s degree in social work, nursing, psychology, or related field, plus one year of experience coordinating services across child welfare systems. Licenses & Certifications: Current state’s clinical license preferred.
