Nurse (Clinical Accountability & Metrics)

This position is located at Chinle Service Unit, Chinle Comprehensive Health Care Center, Division of Nursing Services, Outpatient Nursing Clinics, Tsaile, AZ. Provides case management & care coordination services for patients, including patient assessments, follow-up care, & evaluation activities to support continuity of care & improve patient outcomes in outpatient clinical settings. BASIC REQUIREMENT(S): Education A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education external link, Council on Accreditation of Nurse Anesthesia Educational Programs external link, Accreditation Commission for Midwifery Education external link, or an accrediting body recognized by the U.S. Department of Education external link at the time the degree was obtained. Degree from Foreign Nursing School Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure- For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level GS-09 in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Specialized experience includes performing professional nursing duties that include providing case management and care coordination for high-risk or at-risk patients; assessing patient needs and developing, implementing, monitoring, and evaluating individualized multidisciplinary plans of care; coordinating referrals, specialty care, follow-up services, and continuity of care with multidisciplinary healthcare teams and outside agencies; providing direct nursing care, triage, and clinical interventions; educating and counseling patients and families on disease processes, prevention, treatment, and self-care; monitoring clinical results and patient outcomes; maintaining accurate clinical documentation; and participating in patient safety and quality improvement activities.
