At the Arizona Department of Veterans' Services (AZDVS), we are committed to serving and honoring veterans, service members, and their families by ensuring that they receive the highest quality services so that they can thrive for a lifetime!We are looking for a compassionate Senior Social WorkerYour contribution:• Outstanding Service: Provide the best assistance to veterans to improve their quality of life• Continuous Improvement: Participate in facility meetings, training sessions, and the implementation of the Arizona Management System (AMS) to achieve continuous improvement and efficiency• Team Collaboration: Work with other professionals to use your expertise while improving your knowledge, skills, and abilitiesJob Duties:Plans, organizes and supervises the social services program of the Arizona State Veteran HomeEnsures timely completion of assessments and documentation per federal and state regulations and facility policies and proceduresEnsures compliance with the PASRR (Preadmission Screening and Resident Review) process Supervises subordinate staff to ensure the goals and objectives of the work unit are metPlans, organizes, assigns, and reviews the work of the Social Services staffReviews and evaluates job performance of staff through monthly 1 on 1 coaching sessions and annual performance appraisalsAddresses staff problems and recommend disciplinary actionsInterviews and selects staff Oversees clinically appropriate discharge planning to ensure it is safe and communicated effectively to all involved stakeholdersMonitors Medicare notices to ensure timely delivery to beneficiaries per federal lawCompletes consults for referrals to specialist at the Veterans Administration (VA)Works with residents, families, and staff to ensure that veterans/members are treated in accordance with Resident’s RightsAssists in projecting department budget working within budget parameters and monitors expendituresMaintains a personal caseload of assigned veterans/membersAttends and participates in various meetings and serves on committeesKnowledge, Skills & Abilities (KSAs):Knowledge of:CMS, and State regulatory standardsSocial Services programs and principles of social workGeriatric psychiatric issuesGeriatric disease processesSide effects of psychotropic medicationsPhysical and chemical restraintsCommunity resourcesGovernmental policies and programs that affect our clienteleLong-term care billing standardsMDS systemSkills in:Verbal and written communicationCounselingCase managementResident assessmentDeveloping care plansAnalytical skillsProblem solvingSupervision and managementAbility to:Establish and maintain interpersonal relationships at all organizational levelsUtilize the facility's IT program and maintain and record assessments and care plansMaintain the highest level of confidentialitySelective Preference(s):Master’s degree in social work REQUREDOne year of supervisory experience in a health care settingExperience in long-term carePre-Employment Requirements:Active social work license issued by theArizona Board of Behavioral HealthExaminers upon placement into positionAbility to obtain and retain a valid AZDPS Fingerprint Clearance CardCurrent Negative Tb skin test (Mantoux skin test) within the last 6 months or a YearRequired Skills:SQL ExpertPower BISnowflakeData Analysis Expressions (DAX)Preferred Skills:CGI AdvantageTranslate business requirements into scalable BI and data solutionsRequired Skill:HireRight used for Background Checks and Global ID facial Biomatrix RequiredActive social work license issued by the Arizona Board of Behavioral Health RequiredMaster’s degree in social work Preferred #J-18808-Ljbffr S-R-International-Inc
