Description

• Evaluate patients in either an inpatient or outpatient setting and design individualized treatment plans
• Administer physical therapy interventions focused on mobility, strength, balance, and function
• Collaborate with nurses, physicians, case managers, and other rehab professionals to optimize outcomes
• Educate patients and families on therapeutic goals, discharge planning, and home exercises
• Document evaluations, treatments, and progress in accordance with facility and regulatory guidelines
• Participate in care team rounds and interdisciplinary conferences to ensure coordinated care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) are a plus, but not required

Skills

• Previous experience strongly preferred
• Ability to treat patients with complex medical and mobility issues
• Strong communication and teamwork skills
• Comfortable using digital charting and clinical documentation systems
