Certified Nurse Aide Home Health Aide

As a Certified Nurse Aide Home Health Aide with BrightCare HomeCare, you will be part of a dynamic and compassionate team committed to delivering exceptional care services to our clients. Responsibilities include:
- Provide personalized care services to clients, including personal care, assistance with daily living activities, and support with mobility and transfers
- Conduct vital signs and report any changes to the nurse or supervisor
- Assist with medication management, including administration and documentation
- Collaborate with the healthcare team to develop and implement individualized care plans
- Maintain accurate and detailed records of client care and services provided
- Communicate effectively with clients, families, and healthcare providers to ensure seamless care coordination
- Participate in in-service training and education to enhance skills and knowledge
Working conditions include:
- Work in clients' homes, with varying schedules and environments
- May be required to lift, transfer, or reposition clients, and move equipment or supplies
- Must be able to work independently and as part of a team
- Must be able to effectively communicate with clients, families, and healthcare providers
